Levels and experience are guidelines based on topic-specific experience. All courses support foundational and advanced learning.
About:
This interactive workshop provides practical training in Root Cause Analysis (RCA) for improving reliability, safety, and operational performance. Participants learn structured methodologies for identifying physical, human, and latent root causes, applying common RCA tools, developing corrective actions, and verifying long-term effectiveness through case studies and hands-on breakout sessions.
What You’ll learn:
Explain the principles and objectives of Root Cause Analysis (RCA).
Apply structured RCA methodologies to identify physical, human, and latent causes of failures.
Utilize common RCA tools to investigate incidents and develop effective corrective actions.
Validate improvements through implementation, tracking, and case-based exercises.
Who Should Attend:
Root Cause Analysis Training Workshop is designed for maintenance and reliability engineers, discipline engineers (process, machinery, and fixed equipment), leadership and management representatives, and Health, Safety, and Environmental (HSE) professionals involved in failure investigations and continuous improvement initiatives. Cross-functional team participation is encouraged to strengthen collaboration and improve the effectiveness of root cause analysis across the organization.

Taught By:
Gene Meyer, Mike Cooper
Meet The Instructor:
Gene Meyer
Gene R. Meyer has over 40 years of experience in reliability, metallurgy, mechanical integrity, and process safety in petrochemical and chemical facilities. His experience includes 37 years with owner-user companies, leading global reliability and maintenance improvements, integrating new plants and acquisitions, and investigating process safety and downtime events. He has led reliability improvements across businesses with 40 operating plants and developed and delivered training in FMEA, RCM, and Root Cause Analysis. He holds a BS in Metallurgical Engineering from Missouri University of Science and Technology and is a Registered Professional Engineer in Michigan and Ohio.
Mike Cooper
Michael Cooper has over 35 years of reliability, maintenance, and mechanical integrity experience, with leadership roles at Honeywell, Kraton, Westlake, Syngenta, Koch Nitrogen, Dow / Rohm & Haas, and BP Amoco. At Honeywell ESS, he deployed Root Cause Analysis training for over 200 personnel and helped generate $2 million in annual savings. He holds a BS in Mechanical Engineering from Texas A&M University–Kingsville, an MBA from the University of Houston, and CMRP, CRL, and MLT I certifications.
